Rescheduling, Late Cancellations, No-Shows:

Please note that you will be responsible for 100% of your appointment fee if you miss an appointment or cancel without 24 hours notice. Please do your best to notify me in advance if you are unable to attend your appointment, and I will gladly find time to reschedule.

    **Must have screening and assessment prior to being eligible.

  • $22.50 Per Half hour

    Services to coordinate care with other providers. (i.e. transfer to higher level of care, coordinate needed mental health care, collaborate with other providers/community partnerts for increased and/or continued program success as/if needed).

  • $56.63 Flat rate per session
